Life Events

Call the Benefits Service Center within 31 days of the date of your qualifying life event to enroll or make changes. Please note IRS regulations for birth, adoption or death must be made within 60 days of the event. If you miss this enrollment window, you'll have to wait until the next Open Enrollment period.

Getting Married

Congratulations. Getting married is an exciting time in your life. There are many details in addition to a wedding that you need to consider including your benefits. You may enroll your new spouse in your medical, dental and/or vision plan. If you have an HSA plan, review your contributions, your coverage level may have changed. You may enroll in Accident insurance or add your new spouse. You may want to enroll your spouse in supplemental life insurance if you also have supplemental life insurance through Mohawk.  Check your beneficiaries for your basic life insurance, supplement life insurance (if applicable), 401(k) and your Health Savings Account.

When you go through a divorce, it can be a very stressful time in your life. There are many details you need to think about including legal, financial and emotional. Here are some benefits-related details you should review.

You must drop your former spouse and/or former stepchildren from you medical, dental and/or vision plans. If you have an HSA plan, review your contributions, if you change coverage levels you don’t want to over-contribute. You must remove your former spouse and/or former children from your supplemental life insurance. You must remove your former spouse and/or former children from your accident and/or critical illness insurance. Review your beneficiaries for your basic life insurance, supplement life insurance (if applicable), 401(k) and your Health Savings Account as you may want to make changes.

Having or adopting a baby

Congratulations! Welcoming a child is a wonderful time in life. As you’re navigating the world of a new child, take the time to navigate your benefits to make sure your new addition is protected. Here are some benefits-related actions you may want to take after your child is born or your child's adoption is finalized.

You may enroll your new child in medical, dental and/or vision plan. If you have an HSA plan, review your contributions, your coverage level may have changed. You may want to enroll your child in child life insurance if you also have supplemental life insurance through Mohawk.  Don’t forget to take advantage of the Mohawk Bonding leave.

If you your spouse’s employment status changes and is no longer eligible for benefits coverage with their employer, you can make changes to medical, dental and/or vision coverage. If you have an HSA plan, review your contributions, your coverage level may have changed when adding a spouse. You may also make changes or enroll in accident insurance. You may want to enroll your spouse in supplemental life insurance if you also have supplemental life insurance through Mohawk.  Reminder, employees and spouses on a Mohawk health plan need to complete a biometric screening. It may also be a stressful time if your spouse has lost a job. Don’t forget about Employee Assistance (EA). They have resources to help you with home and work life. If your spouse’s employment status changes and is eligible for benefits coverage with their employer, you can make changes to your medical, dental and/or vision coverage. If you have an HSA plan, review your contributions to ensure you are not over contributing.  If your spouse has access to other group medical coverage through their employer a spousal other coverage surcharge will be applied in addition to your premium cost. $28.85 per week or $125 per month

If you have a dependent under age 26 that is no longer has access to benefits coverage, you can make changes to your medical, dental and/or vision coverage. You may also make changes or enroll in accident insurance.
